Forms and Applications are Organized by Village Departments.

PLEASE NOTE:  The fees listed on the applications below will be the most accurate.  If you obtained your rates/fees from the codified Ordinances, please remember these Ordinances are only updated on an annual basis and may not reflect the most recent changes.  Contact Village Staff for the most accurate information.

Water/Sewer Service Application  Applications are accepted Monday - Friday 8:00-4:30 PM.  All applications must be submitted in person AND all applicants on the account must be present with a state / government issued - Photo ID.  There is a $40.00 (forty dollar) application fee that is payable by CASH or CHECK ONLY. THE APPLICATION FEE IS NON-REFUNDABLE.  CREDIT/DEBIT CARDS ARE NOT ACCEPTED AT THIS TIME. 

Direct Debit Authorization Form - be sure to bring a voided check with the completed application. 

Pool Fill Adjustment Request - Please complete this form and submit it to the Village Office.  Please note: only 1 pool adjustment is allowed per year.
